MEDICAL RELEASE: I hereby render permission to the staff of the Rappahannock Youth Orchestras, Inc., D/b/a Rappahannock Youth Symphony, to have my child treated in a medical emergency situation, if I cannot be reached within a reasonable period of time.

TUITION AGREEMENT: I agree to pay the full amount of tuition: $300 for the whole season; $175 for half season; $175 per additional sibling. If financial assistance is needed, I agree to provide required paperwork to the Board of Directors by October 1, 2011.

Absence Form

Procedure: Students must submit the following form to the Executive Director at least one week prior to the requested absence date. Requests for absences will be reviewed on a case by case basis. Completion of the form is not a guarantee of approved absence.

In case of emergency or unanticipated absence, please immediately call or text Megan Bevill, Executive Director, at 443-852-2205. A letter of explanation should be submitted at the following rehearsal.
